I certify the information on my application is true and correct to the best of my knowledge and any misrepresentation or omission on my part is cause for rejection of my application or termination of employment if I am hired. I agree that this disclosure and authorization the original, faxed, photocopied or electronic (including electronically signed) form will be valid for any requests for information that may be requested by the Company.


Drug/Alcohol: I hereby authorize any person or company for whom I have worked, or to whom I have applied to work in the past three years to release the date and type of any drug test with a positive result, any alcohol test with a concentration of 0.04 or greater, or any refusal to take a test. This list should include all tests required by the FMCSR or conducted by the Company under their Company policy, and any state that has a drug/alcohol database search. I authorize the release of all information concerning my referral to a Substance Abuse Professional (SAP) including all records pertaining to my evaluation and treatment.

By signing below, I give CalArk International, Inc./CalArk, Inc. (CAI/CAT) the right to investigate all references and to secure additional information about me, if job-related. I release CAI/CAT and its representatives from liability or damages for seeking such information and I release all persons, corporations, or organizations from liability or damages for furnishing such information. A copy of this page serves as my authorization to seek/provide this information. I agree to sign all documents and consent that CAI/CAT deems necessary to verify the facts provided in this application. I give my consent to and release CAI/CAT and its representatives from liability and damages to respond to any inquiries made about me as part of a reference check by any subsequent employer, or potential employer, if I leave my employment with CAI/CAT.



I understand and accept that as part of the application and employment process, and/or during employment with the Company, the Company may ask me to submit to physical examinations which may include testing for alcohol and drugs, and/or be fingerprinted, all in accordance with applicable law. By signing this application, I agree to submit to those examinations and release all persons and companies from all liability arising out of any examinations, tests, and finger printings. I further agree that the examining person may disclose to the Company or its representatives the results of any examinations and tests.



I understand that any offer of employment is contingent upon my ability to produce documentation verifying my identity and legal authorization to be employed, as required by the Immigration Reform & Control Act of 1986 (IRCA).



If employed, I agree to conform to all the Company’s policies, practices, and procedures and acknowledge that the Company may change, interpret, withdraw, or amend those at any time, at the Company’s sole option and without any prior notice to me. I consent and agree that the Company possesses the right to search my personal property located on the Company’s property, along with the Company’s desks, closets, et cetera, for the purpose of investigating possible violations of the Company’s rules/policies. This consent also includes access to my telephone conversations and e-mails or other types of electronic communications.



I understand and agree that any misrepresentation by me in this application will be sufficient cause for cancellation of this application and separation from CAI/CAT’s service if employed. Furthermore, I understand that just as I am free to resign at any time, for any reason, with or without prior notice, CAI/CAT reserves the right to terminate my employment at any time, for any reason, with or without prior notice. I understand that no representative of CAI/CAT has the authority to make any verbal or written assurances to the contrary. I recognize this employment relationship to be an at-will relationship and not for a specific period of time. This application represents the complete and total expression of the intent of the parties and may not be modified except by writing duly executed by the undersigned and the President of the Company.
